From "Declaration for an Original Invalid Pension", 1890:

State of Wisconsin County of Crawford, ss. On this 13" day of June A. D. one thousand eight hundred ane ninety --- personally appeared before me Notary of the Public a COURT OF RECORD within and for the County and State aforesaid Reuben Cooley aged 60 years, who, being duly sworn according to law, declares that he is the identical Reuben Cooley who was ENROLLED on the 14" day of Aug, 1862, in company "D" on the 31" regiment of Wis. Infin commanded by O. B. Thomas and was honorably DISCHARGED at Madison Wis on the 6" day of July, 1865; That his personal description is as follows: Age 60 years; height 5 feet 10 inches; complexion light, hair light; eyes blue That while a member of the organization aforesaid, in the service and in the line of his duty at or near Murfreesboro in the State of Tenn. on or about the day of Jan'y, 1864, he contracted "Rheumatism," also at Columbus Ky in summer of 1863 he contracted Chronic Diarrhaea and resulting "Piles"
